How can I set up an appointment?
To set up an appointment over the phone, simply call (763) 421-7420 during our
office hours. To request an appointment over the web, just click here.
Do I need to arrive early for my appointment?
It is not necessary to arrive early if you have been seen at our office before. However, if you are a new patient, it is recommended to arrive five to ten minutes early to register.
Do I need a referral to be seen at your clinic?
Many health insurance providers issue identification cards with your primary care clinic printed on the card. With few exceptions, if there is a clinic name listed on your card, a referral is needed. The safest answer is to always contact your insurance company before being seen here. View a list of insurance plans that we are participating providers for.
If it is an emergency, can I be seen today?
In emergency situations, we will work patients into our daily schedule. If you have an emergency that is not during our normal office hours, please call our office at

When is my copayment due?
